Advanced Primary Care Plus is a subscription based primary care model where employers pay a monthly subscription fee for their employees and families to have unlimited access to their primary care provider.

● Establish strong doctor-patient relationships by providing timely, comprehensive and personalized care

● Convenient access-same/next day appointments

● Minimal wait times upon member arrival

● Extended unhurried office visits (typically 30-60 minutes) where the patient feels heard and understood

● 24/7 availability via text, phone email or video

Contracting Process


● Provider contracts with GSD Practice Management

● GSD Practice Management Enrolls the provider with Apaly

● GSD Practice Management the doctors with the monthly membership reports

● GSD Practice Management reaches out to the new members and schedules the first visit

● GSD Practice Management uploads the member visit to the Apaly platform

● Provider receives monthly ACH payment of $70 PMPM
